The first of its kind, the digital Better Living Toolkit, developed by the Arthritis Foundation and sponsored by Lilly, is designed to help you understand your disease and take positive steps toward living a life of Yes. From a unique, interactive Health Tracker, to goal-setting worksheets and materials to help you improve communication with your doctor, these tools and resources will empower you to make informed decisions about your treatment.
RAPID-3© Health Tracker
The Arthritis Foundation’s easy-to-use Health Tracker will help you calculate a Rapid-3 score, commonly used by rheumatologists. With this tool, you’ll provide your doctor with important information not available from blood tests, X-rays or any other source other than you. You’ll assess four key areas:
- Ability to easily perform tasks of daily living
- Pain levels
- Sleep patterns
- Emotional health (feelings of stress, anxiety or depression)
When your score is calculated, you can enter your numbers each week in a tracking sheet (also provided in the toolkit) and create a graph of your progress over time. By sharing this information with your doctor at each visit, you’ll both be able to monitor the effectiveness of your treatment plan.
Tools to Communicate With Your Doctor
Everyone has a different experience with arthritis, so honest and thorough communication with your doctor is key to receiving the right treatment. Our communication resources include:
- A personal health assessment to help your doctor understand how you’re feeling
- A worksheet to help you prioritize the questions you want to ask at each appointment
- An easy-to-understand glossary of arthritis terms – available online and downloadable in the toolkit
These resources will help you prepare for an effective office visit as you and your doctor work together to develop your personalized treatment plan.
Goal Setting Tools
If you have arthritis, setting meaningful goals – and making an action plan to achieve them – can help you get back to doing the things you love. You’ll learn what SMART goals are – and how to achieve them – with our goal setting resources, such as:
- SMART goal-setting worksheet to keep you focused and moving forward
- SMART action plan worksheet to help you achieve your goals
- SMART goal contract to help you make a promise to yourself that you’re going to achieve your goals
Sharing your goals with your doctor can help make them as effective as possible. He/she can recommend steps you can take for success, like exercise or changing your medications.
